MEDICAL FORMS:
Medical forms and other pertinent personal data (Allergy Data Forms), etc. are a state requirement. Please make sure that through the application process and that annual medical form up-dates are provided to assure that there is a no lapse in your child care.
Without such forms we can not allow a child to attend the school or be under our care.

We are bound by all requirements of HIPPA to ensure the security of your child's personal medical information. The same is true for all personal data and information you may share with us in the process of caring for and safeguarding the welfare and health of your child.
